HIIN Falls Program: Re-Energizing Prevention Practices

This full-day program will focus on best practice approaches to fall and injury prevention. The program will be led by Patricia Quigley, PhD, a nationally renowned expert in clinical practice innovations in patient safety, nursing and rehabilitation designed to promote independence and safety.

First priority will be given to MHA/HRET Hospital Improvement and Innovation Network (HIIN)  hospitals.  Registration will be offered on a space available basis to all other Maine Hospitals.

CMS Proposed Changes To Discharge Planning & IMPACT Webinar

This program will focus on the Improving Medicare Post-Acute Care Transformation Act (IMPACT) and the CMS hospital proposed changes to the discharge planning standards. This session will be beneficial for hospitals as well as Critical Access Hospitals (CAHs).

Bridging the Post-Acute Care Gap To Reduce Readmissions Web

The IMPACT Act helps assess the health status of individuals as they transition between acute and post-acute care settings. If these care settings are not “speaking the same language” by collecting and reporting on different health criteria, this can hurt quality measures and contribute to readmission penalties in both care settings. This presentation will address this issue and much more.
